Our business hours are Monday through Friday, 8:00 am - 5:30 pm.

What should you bring to your appointment?

Please remember to bring the following with you to your appointment at OrthoMed Center:

  • Driver’s License or valid identification
  • Insurance information
  • Completed New Patient Packet (for patients new to the practice)
  • Referral or authorization (if required by your insurance plan)
  • X-rays, MRI, and /or CT Scan images, if already taken
  • Written reports of all test results, including lab, X-ray, MRI, or any other     imaging test pertaining to your visit    
  • List of medications you take including name, dosage, and frequency
